The Project Request Form is in PDF form (that just means you need Adobe Acrobat Reader to open it.) If you don't have Adobe Acrobat Reader, it is free. To get a copy, just click on the link below. When you click on it, your internet browser will automatically open to the correct page for downloading a FREE copy of the program. Just place your cursor on the blue hyperlink at the bottom of this paragraph, then left click on it, and follow the instructions on the webpage that opens. After downloading the program, you will be able to open the Form. Get Adobe Reader |
The Form can be filled out on your computer in Adobe Acrobat Reader, and then click on the “Submit Form” button in the upper right-hand corner. It will automatically be sent to our Project Coordinator unless you have a web-based email system. If you use a web-based email system, or if you wish, you can fill it out on the screen, save it to your computer, and then send it as an E-Mail attachment. You may also print it out, fill in the blanks and mail it back, any way that works for you will be fine with us. We realize that not all questions on the Form will necessarily apply, so don’t worry about leaving some blank. |
