*The Form can be filled out on your computer in Adobe Acrobat Reader, and then click on the “Submit Form” button in the upper right-hand corner. It will automatically be sent to our Project Coordinator unless you have a web-based email system. *If you use a web-based email system, or if you wish, you can fill it out on the screen, save it to your computer, and then send it as an E-Mail attachment. *You may also print it out, fill in the blanks and mail it back, any way that works for you will be fine with us.We realize that not all questions on the Form will necessarily apply, so don’t worry about leaving some blank. |
